Originally released as Volt 412 on September 15, 1965. This release features a more muted color scheme for the front cover, and this is not due to sun fading. Some copies come with a yellow hype sticker on the shrinkwrap advertising "Flashback – Original Artists * Original Recordings".

Spine refers to the title just as "Otis Blue", and booklet folds out to a 4-panel display of the original back cover to the LP.
